Apple Valley will be doing a mass (tier 1) vision screening  for grades K, 1st, 3rd, and 5th on September 14th. If your student wears glasses or contacts please help your student(s) remember to bring them to school for the screening. Per Utah Law (UCA 53G-9-404) vision screening is provided yearly for all students in grades pre-K, K, 1st, 3rd, 5th, 7th, and 10th. If a student does not pass the screening, a referral letter will be emailed or mailed home. Nebo School District provides a basic distance (tier 1) vision screening for at least grades  PK, K, 1, 3, 5, 8, and 10. Tenth grade students may be screened as part of their driver’s education. In addition, NSD nurses may do a (tier 2) vision screening for those referred by parents, teachers, special education, or others. On referral, a vision questionnaire is completed by a teacher or parent (if referred by parent) and given to the school nurse. The school nurse may screen for distance, near, eye focusing or tracking problems, color vision deficiency, and/or convergence insufficiency. A referral to an optometrist or ophthalmologist may be given to the parent if determined by health condition, vision questionnaire or vision deficiency determined by the screening (UDOH, 2019). School screenings are coordinated by the school nurse who may use trained parent volunteers and school staff to assist, or may request assistance from a State approved vision screening provider. The results of the vision screening may be shared with other educators and healthcare professionals to provide appropriate services within the school. If your student does not pass the screening, a referral letter will be sent by mail or email. Please inform your child’s school if you do not want your student vision screened, and complete the "OPT OUT" form. School vision screening is not a substitute for a complete eye exam and vision evaluation by an eye care professional. If you are concerned that your student has a vision issue it is recommended to schedule an appointment with an optometrist or ophthalmologist. If you do not want your student’ vision screened, the “OPT OUT” form will be required each year.
