Utah Give Kids a Smile will be holding their annual event on February 29, 2020 at the Utah College of Dental Hygiene in Orem. This event provides free dental exams and procedures for qualifying low income students age 5-12. For students to participate in this free event parents need to complete the Heath history and consent form and get them faxed in by the deadline. The deadline for completed forms to be faxed in is February 7, 2020. Forms are in the front office.  Parents can return the completed forms to our school nurse, Alicia Milberger, and she will fax them to Give Kids a smile.
